Through individualized consumer-directed services and integration of community resources, Aurora's skilled Registered Nurses focus on the mental, physical, and environmental aspects of health, thereby promoting well being through coordination of care of both individual and organizational services.


  • Conduct nursing assessment during admission process by identifying consumer health status, recognizing emotional, physical and psychological programs, teaching needs and the consumer's expectation for rehabilitation and discharge planning
  • Identify consumer needs on admission, determining appropriate nursing intervention, communicating plan of care verbally and in writing
  • Evaluate consumer and family/provider needs, altering nursing intervention as appropriate and collaborating with physician, family, and service providers to effect changes in care
  • Anticipate and take proper action in emergency situations
  • Develop consumer-centered goals and incorporates them in the written nursing care plan for each assigned consumer
  • Demonstrate flexibility and adapt to changes caused by emergencies in staff, schedules, and consumer needs
  • Provide skillful, thorough, organized nursing care to consumers and demonstrate care through written notes and care plan, including consumer responses to nursing intervention
  • Establish priorities, delegate appropriate tasks to implement nursing care
  • Evaluate and assist in care of consumers assigned to non-R.N. personnel
  • Administer medications and treatment safely and accurately to meet the consumers physical and emotional needs
  • Report physical and emotional reactions to medications and treatments, and document in the consumer's medical record
  • Coordinate discharge plans as needed
  • Identify and utilize appropriate community resources to facilitate the delivery of quality consumer care
  • Provide after hours support
  • Teach consumer and/or his/her family/residential provider when providing services
  • Document all consumer education in the medical record and in the consumer care plan.
  • Follow physician orders for procedures.  Review and interpret physician orders to include frequency and anticipate duration of service, evaluation of the recipient's needs and preferences. May administer medications from a multi dose container or by injection at the direction of a psychiatrist or another physician.
  • Report to consulting psychiatrist and Clinical Coordinator and document in chart adverse drug reactions and potential medication conflicts when drugs are prescribed by more than one physician
  • Coordinate with Case Manager and Clinical Coordinator identification of the actual providers of the services (CSP or non-CSP). Include appropriate CSP follow-up.
  • In collaboration with Case Manger provide or coordinate physical health care services as needed.


  • Must be licensed to practice professional nursing in the State of Wisconsin
  • Must meet qualifications of Home Health R.N. regulations as per Wisconsin Administrative Rule
  • Knowledge of Home Health regulations
  • Associates Degree Required and at least 2 years of expereince 
  • Acceptable Background Character Verification
  • Valid drivers license and acceptable driving record
  • Experience with disability groups
  • Demonstrate proficiency in computer software applications such as Excel and Microsoft Word
